Impact TCPI PTN DPC Grant Project

PRACTICE TRANSFORMATION GRANT

$15 MILLION IN GRANT FUNDS TO HELP PRACTICES TRANSFORM INTO VALUE BASED CARE MODEL-DPC.

The data will be de-identified and aggregated by the not for profit COSEHC and reported to CMS. The goal is to get Direct Primary Care recognized as an advanced Alternative Payment Model under MACRA so that Medicare could pay for DPC memberships for Medicare beneficiaries in the future. That per member per month payment does not exist yet but the hope is that the data collected through this project will persuade CMS to do so.

*THE ABFM AND ABIM ARE SUPPORTING PARTICIPATION IN THIS AS A MOC ACTIVITY FOR PART 4

* A FINANCIAL INCENTIVE OF SEVERAL HUNDRED DOLLARS PER YEAR WILL BE PROVIDED FOR PARTICIPATING IN THE PROJECT

*PRACTICES ENROLLED RECEIVE FREE TOOLS, RESOURCES, & TRAINING BOTH ONLINE AND IN PERSON

*DATA EXTRACTION WITH REPORTING WILL ALLOW YOU TO MEASURE THE HEALTH OF YOUR PANEL AND MAKE YOU APPEALING TO EMPLOYERS FOR DPC CONTRACTS
