Direct Primary Care Medical Home Certification 

 

   Become a Certified Direct Primary Care Medical Home.

 

    Fill out a short questionnaire about characteristics of your CURRENT practice.  If you meet the criteria for a Direct Primary Care Medical Homes Center of Excellence then we will either email or mail you a certificate that is valid for one year.  This primarily will be used to determine eligibility for the Access Direct Patient Assistance Program (ADPAP).  This program, which is a joint venture of the DPCMH Association and NeedyMeds, provides lower income patients with grant funded DPC memberships.  It is important that the patients enrolled in this program have a standard expectation of their DPC Medical Home experience and that the practices meet certain minimum criteria to participate.  With the ADPAP, grants are provided to eligible certified practices to cover the complete costs of DPC membership for awarded patients.   There is not a complicated NCQA type application. 

There are only 11 questions required that need to be answered annually to continue certification.
